3. Pay by mail: Mail a check payable to Knights of Columbus Council 4050 and send with this form: Payment must be received by Sept 18th.

Instructions-

There are three options for ordering the Chicken Dinner-
1. Online using this site (see the Online Ordering Tab below). You can pay by credit card at time of online purchase so you don’t have to pay at pickup.
2. Email kofc4050@gmail.com. Please provide the information requested on the flyer in your email. You will have to pay at the time of pickup.
3. Printing out the form (found at the bottom of the page by clicking the download button), filling it out, and sending the filled out form and check (ONLY). The check is payable to “Knights of Columbus Council 4050”. If you don’t have a printer, simply provide required information legibly on a piece of paper and return with payment. Please mail it to:
